The title is inspired by Act 1, Scene 2 of Shakespeare's play Julius Caesar, in which the nobleman Cassius says to Brutus: "Men at some time were masters of their fates, / The fault, dear Brutus, is not in our stars, / But in ourselves, that we are underlings." Author John Green was inspired to write the book after working as a student chaplain in a children's hospital, and it is dedicated to his friend Esther Earl, who died of Thyroid cancer in 2010, age 16.
